Firearm Evaluations
FOID (Firearm Owner’s Identification Card) forensic psychological evaluation is usually requested when the person’s application for the firearm card was denied due to prior psychiatric inpatient treatment.
We provide, complete, and submit all required forms directly to the Illinois State Police, Firearms Services Bureau, Appeals Section. You should have your psychiatric records sent to us for review prior to the evaluation.
Firearm forensic psychological evaluation takes approximately 4 hours of face-to-face assessment and can be completed in-person or via videoconferencing.
